Living a full and productive life with heart disease is very achievable today. Our Cardiac Rehabilitation Phase II 12-week program is designed to help you regain confidence and quality of life through:

The program is specific to patients who have had coronary artery stents, bypass surgery, heart attacks, heart valve repair, chest pain (angina) and heart transplants. Your individual, physician-prescribed program will be guided by a cardiac care RN and an exercise physiologist.

Exercise maintenance program
An exercise maintenance program, designed for individuals who want to increase their agility, flexibility and improve their quality of life and independence will be offered at the Live Well Be Well Center.
