Our Transitional Care Unit (TCU) is a licensed swing-bed unit providing short-term orthopedic, neurological, and medical rehabilitation. Our TCU provides care for patients who require continued therapy and nursing care to achieve the independence needed to return home after hospitalization. Our twelve-bed facility is staffed by an interdisciplinary team consisting of physical therapists, occupational therapists, speech therapists, licensed nurses and certified nursing assistants.
Our services include:
- 24-hour skilled nursing care
- Physical therapy seven days a week (if ordered)
- Occupational therapy up to five days a week (if ordered)
- Speech therapy as needed
- IV therapies
- Social services for emotional support and discharge planning
- Activity therapy
- Pharmacy consultations
At Adventist Health Sonora, our focus is on helping patients gain independence rather than being "taken care of." Our patients practice activities of daily living such as getting dressed and caring for their personal hygiene. Our goal is to help our patients lead a productive and satisfying life regardless of physical challenges.
