What is a Healthcare Directive?
A health care directive (some call it an Advance Healthcare Directive) is a form that makes your health care wishes known to your family and physicians if for whatever reason you become unable to state them yourself.
This directive will allow you to appoint another person to be your health care "agent". The “agent” you designate will be authorized to make decisions for your medical care when you become unable to.
- This document also allows you to specifically state what treatments you would like or not like and other detailed healthcare instructions.

At Adventist Health Ukiah Valley, these forms are available at the Admissions Office and Chaplain’s Office. Also any nursing station can make sure you receive a Healthcare Directive form.
